รูปแบบของเกมส์ NeverDead เป็นเกมส์แนวมุมมองบุคคลที่สาม หรือ TPS ซึ่งในการเล่นจะมีทั้งแอคชั่นจากการใช้อาวุธโจมตีในระยะประชิดและปืนสั้นรัวคู่กัน บรรยากาศภายในเกมส์จะใช้ภาพของเมืองที่โดนเหล่าอสูรบุกถล่มทำลายจนเละ และเต็มไปด้วยคำสาปเชื้อโรคร้ายต่างๆ แต่ตัวเอกของเรื่องนั้นได้รับเอาพลังพิเศษอย่างหนึ่งมาจากและผลนั้นทำให้ตัวเขากลายเป็นปีศาจ มีความสามารถเหนือกว่ามนุษย์ทั่วไป และที่พิเศษที่สุดก็คือ ความสามารถที่เป็นอมตะฆ่าไม่ตาย



แม้ว่าร่างกายจะโดนทำร้ายมากแค่ไหน หรือจะขาดกระเด็นไปยังไง ก็สามารถนำกลับมาต่อใหม่ได้อีกครั้ง นั้นคือภาพที่เกมส์ NeverDead แสดงออกมาให้เห็นถึงความสยองของตัวละคร และเหล่าสัตว์ประหลาดทั้งหลายภายในเกมส์ดูสยดสยองและน่ากลัามากๆ เน้นด้านขนาดให้ใหญ่แบบเห็นแล้วหลอนนำมาก่อนเลย อย่างเดียวที่ดูไม่สยองคือหญิงสาวหน้าใสที่ทำหน้าเป็นคู่หูของตัวเอกเรา ชิ้นส่วนของร่างกายที่ขาดกระเด็นออกไปนั้นจะสามารถเคลื่อนไหวและโจมตีได้ เหมือนกับฮีโร่สไตล์ซอมบี้ อย่างไรก็ตามระบบภายในเกมส์ยังไม่มีการเปิดเผยรายละเอียดและลูกเล่นมากนัก โดยตัวเกมส์จะลงบนเครื่อง PS3 และ Xbox360
